Abstract
Bone cement used in Total Hip Arthroplasty (THR) and Total Knee Replacement (TKR) procedures has profound haemodynamic effects, including hypotension, cardiac arrest and fat embolism. We studied the haemodynamic changes in 50 adult ASA physical status II patients belonging to either sex and with no evidence of severe cardiovascular disease undergoing THR or TKR without tourniquet, after approval by hospital ethical committee and written informed patient consent. After preloading with lactated Ringer's solution 10-15ml/kg of body weight, subarachnoid block was given with Bupivacaine 0.5% heavy 15mg intrathecally. Systolic, diastolic, mean arterial pressure, heart rate and SpO2 were recorded just before insertion of bone cement and then every minute for next 10 minutes after insertion of bone cement No statistically significant haemodynamic changes from baseline were observed after bone cement insertion in any patient. In conclusion, bone cement does not have any adverse haemodynamic effects in patients undergoing THR and TKR, if adequate volume replacement is assured and proper surgical technique adopted.
